Glaucoma can cause permanent vision loss, but modern treatment provides several ways to lower eye pressure and protect the optic nerve. Today, glaucoma care can be more proactive. For eligible patients, treatment may begin with an interventional option instead of depending exclusively on daily prescription eye drops.
Detecting Glaucoma Before Vision Is Lost
Glaucoma develops when the optic nerve becomes damaged. Elevated pressure inside the eye is one of the most important risk factors, although glaucoma can also occur at lower pressures.
The disease may progress for years without causing pain or obvious symptoms. As peripheral vision gradually declines, the brain can compensate for the missing information. This can make changes difficult to recognize without comprehensive eye exams and glaucoma testing.
Why Eye Drops May Not Be the Entire Answer
Prescription eye drops can effectively lower eye pressure. However, they must be administered correctly and used consistently. Missed doses, medication costs, side effects, and difficulty placing drops in the eye can interfere with treatment.
When appropriate, an ophthalmologist may recommend an interventional treatment that does not depend on a daily medication schedule. Laser treatment may be used before eye drops, alongside medication, or when drops are not providing sufficient pressure control.
DSLT as an Early Treatment Option
Direct Selective Laser Trabeculoplasty, commonly called DSLT, is an automated, noncontact laser treatment that helps improve the eye’s natural drainage system. When fluid drains more effectively, pressure inside the eye may decrease.
Traditional Selective Laser Trabeculoplasty requires a specialized lens to be placed on the eye while the ophthalmologist manually directs the laser. DSLT uses advanced imaging and eye tracking technology to identify the treatment area and deliver laser energy without a device touching the eye’s surface.
Potential advantages of DSLT include:
- No incisions or stitches
- No contact with the eye’s surface
- Automated laser targeting
- A brief outpatient treatment
- Minimal disruption to normal activities
- Reduced reliance on glaucoma medication for some patients
DSLT may be considered for certain patients with open angle glaucoma or ocular hypertension. However, results vary, and some patients may continue to need prescription eye drops following treatment.
Expanding Treatment With MIGS
Minimally Invasive Glaucoma Surgery, or MIGS, describes several procedures that help lower eye pressure while causing less disruption than traditional glaucoma surgery. Depending on the procedure, MIGS may improve fluid drainage, create an additional drainage pathway, or reduce fluid production.
MIGS is commonly considered for eligible glaucoma patients who are also planning cataract surgery. Potential benefits may include small incisions, a relatively brief recovery, better pressure control, and less reliance on medication.
The appropriate procedure depends on the type and severity of glaucoma, previous treatment results, and the patient’s target eye pressure. Patients with more complex or advanced disease may require traditional glaucoma surgery.
